Bought the 200-GC package? After the form above, also complete this short second form. It starts by asking for your company name and your PayPal transaction ID so we can match it to your file.
Email your documents to prequal@caconstructionleads.com, with your company name in the subject line:
W-9 · certificate of insurance · CSLB license · EMR letter · written safety program (IIPP)
You will get an automatic reply listing what we received and what is still outstanding, so you always know where you stand. If a file is too large, send it in two emails or share a link.
The day the last of those two things arrives — not the day you paid. Time spent waiting on you does not count against the 15 days.
You have 30 days from your payment date to send both. After that we close the order with whatever we were able to deliver, and we cannot accept refund requests.
